我觉得这里少单词
that the individual or his or her personal representative will rely on the diagnosis.
我没找到这句话的谓语动词，也许应该在少单词的地方吧
要么就是“so……that……”结构，却只有that没看见so
还望高手指教！
另外，我记得rely on是依赖信任的意思

起初我也是这么认为的，但是细细一想又不对了，如果这样的话'that the individual or his or her personal representative ' 这个从句又缺乏谓语了，意义上也将不通。
本来这句话就是从一般的教科书中摘抄出来的，而不是考语法，没必要为要按照TOEFL的要求仔细分析。

编书的并没错。这段话可减写为：Communicating to the individual a diagnosis when/if it is reasonabley foreseeable that the individual will rely on the diagnosis.这段话似乎是从医务管理方面的专业书上来的，带法律色彩。
“his or her personal representative“ 与“the individual” 并列，可去掉。这是比较严谨的法律写法。
“identifying a disease or disorder as the cause of symptoms of the individual” 为非限制性的，可去掉。
“in circumstances in which”相当于“when or if"。
“that the individual or his or her personal representative will rely on the diagnosis.”为限制性的，不可去掉。
一段话不一定就要是一个完整的句子。

这不是一个完整的句子，应该是“名词从句”（我的英语语法忘得差不多了，不知准确的说法）。主体是“Communicating to the individual or his or her personal representative a diagnosis”，其后的“identifying a disease or disorder as the cause of symptoms of the individual”修饰“diagnosis”，余下的“in circumstances in which it is reasonabley foreseeable that the individual or his or her personal representative will rely on the diagnosis.”是状语从句。
也就是说：在“in circumstances in which...”的情况下做“Communicating to...”这件事。
