This is a metaphor, refering to a game being played.Thus the earth's last chance is being played now. So qu'elle(she) refers to the game. se joue means the game is played. It is pretty hard to translate without more context. Would be nice if you poseted the whole song

'C'est maintenant qu'elle se joue' is an expression that refers to earth's last chance. Here, earth's last chance is expressed as a sort of game that is being played (jouer). If the game is lost, then earth dies. If the game is won, earth lives and remains beautiful.
The word 'se' refers to 'chance' (or game metaphoricaly). Chance is a feminine word, hence 'elle'.
