1.配偶的spouse当然是你，2.无所谓，3.需要，4.最好改户口本，5.只要邮局能够正确寄到就可以。//你的提问方法有点有趣，只不过浪费了版面资源。:)

Bingle, 万分感谢你及时的回答。对1，你的意思是不是副申请人在填写申请表第二页spouse栏时，应该填的仍是副申请人的资料？我填表填得头都的了。

晕倒，看来我仍然没有讲清楚。主申请人的配偶栏，填得是副申请人的资料；副申请人的配偶栏，填得是主申请人的资料。（参见FAQ）//BTW，主申请人和副申请人的材料以及准备方法，基本上完全一样，真正体现了主副平等。
